Runner’s knee, or PatelloFemoral Pain Syndrome (PFPS), refers to pain in the front of the knee and around the patella or kneecap. It is a prevalent issue among athletes who participate in sports that require frequent knee bending, including running, cycling, and jumping. The pain is typically felt during activity but can also occur during everyday movements such as walking up and down stairs or squatting.

The causes of PFPS can be numerous and interlinked. Overuse from repetitive forceful motions may irritate the kneecap’s underlying cartilage and lead to pain. In contrast, misalignment of the kneecap due to muscular imbalance in the quadriceps, hamstrings and hip muscles, as well as foot issues like overpronation,  can increase stress on the kneecap and further compound the condition. Runner’s knee can be caused by various factors, with variations based on each person. Overuse and repetitive stress can often be found among athletes involved in sports that involve running, jumping or direct impact on their knee. Misalignments or mechanical issues that place additional stress on the patellofemoral joint – such as an incorrectly aligned kneecap or poor biomechanical form – also play a role. Ultimately, this leads to pain and inflammation within this joint system. Muscle imbalances also play a crucial role. Weak quadriceps, hip flexors, and gluteal muscles may lead to inadequate support and tracking of the patellar tendon, exacerbating stress on the knee. Tight hamstrings and calves may further pull it out of alignment. Targeted exercises can address such imbalances that contribute both to treating PFPS and helping to prevent its development.

Prevention of a runner’s knee requires addressing all risk factors that contribute to it, particularly strengthening muscles around the knee and hip as well as increasing flexibility of lower body muscles (particularly hamstrings and quadriceps ) so as to alleviate tension and promote better movement patterns. Wearing appropriate footwear tailored specifically for your foot type and gait can also help avoid mechanical issues that contribute to this condition. Athletes should also follow a progressive training plan so their bodies have time to adapt. Integrating stretching and strengthening routines into an athlete’s training schedule is of utmost importance. Exercises such as squats, leg presses, and lunges are beneficial but must be performed with the correct form to avoid additional strain on knees. Furthermore, engaging in low-impact cross-training activities like swimming or cycling may help maintain cardiovascular fitness without overwhelming knees.

Initial treatment for the runner’s knee should consist of rest and ice to reduce inflammation and ease discomfort. Physical therapy is often an invaluable resource, with physical therapists designing personalized programs that combine strengthening, flexibility and bracing exercises as well as advanced biomechanical assessments for targeting stressful movement patterns that lead to knee stress. Targeted exercises may prove highly effective. Manual therapy techniques such as massage and mobilization may also be utilized to improve patellar tracking and ease pain. Orthotics or supportive tapering might also be recommended to correct alignment issues and decrease strain on the knee joint.
